Sports Physicals for Foster Families

We are excited to partner with Victory Sports Rehab and Kinwest Medical Clinic, Inc to bring you sports physicals for foster families. This event is open to any current foster family (any agency) and is offered free of charge. We do ask for a $10 donation for biological and adopted children also in the home. If that will be a hardship for your family, please email Kayla North at info@tapestryministry.org.
Students may NOT come without their foster parent (or another medical consenter). We do NOT provide forms for the physical. You will need to verify with your school what form is required and bring it with you to the appointment time. Failure to do so may forfeit your appointment time.
